For the safety of all attendees with and without fur, all dogs must be currently licensed with proof of their Rabies (if over 4 months of age), Distemper, and Bordetella (kennel cough) vaccines. Should an incident occur, you must be able to produce proof in a reasonable amount of time.
Do not bring your dog if they appear to be feeling unwell, suffering from vomiting or diarrhea within the past 48 hours or you suspect that they are suffering from a contagious condition. Dogs should be healthy and fit enough to take part in activities. Enter them based on your knowledge of your dog, his/her fitness and veterinary history. Please follow any instructions you are given closely.
Please use good judgment regarding the temperament of your dog so that this event will be safe for all attendees. Event organizers reserve the right to eject any aggressive or obviously ill animal. Female dogs in heat are not permitted.
Retractable leashes and electric collars will not be allowed. Please keep your pup on a leash, no longer than six feet in length, at all times other than when in the fenced in Off-Leash, Lure Course or Ball Pit areas. The welfare of your dog is your responsibility throughout the duration of the event and they MUST NOT be left unattended at any time. Dogs should have a collar and a tag with their owner’s name, phone number address inscribed on it.
Please be sure to clean up after your dog.
By participating in the dog Lure Course, Ball Pit, and Off-Leash areas and other Bark & BBQ related activities, you acknowledge and are aware of the inherent risks of injury and/or death to animals and people involved in the recreational activity of canine sports, including without limitations, risks due to dog bite, the use of equipment, or infectious disease. You are voluntarily engaging in the dog Lure Course, Ball Pit, and Off-Leash areas as recreational activities with the knowledge of the risks of injury, death, and damage that may result from participation.
I, the undersigned, understand that participation in the dog Lure Course, Ball Pit, Off-Leash areas and the Bark & BBQ event is not without risk to my dog(s) or myself. I CERTIFY that I am the actual owner of the dog(s) registered to participate under my name, or that I am the duly authorized agent of the actual owner.
In consideration of the acceptance of this entry and the opportunity to participate in events held by Milwaukee Radio Alliance LLC and OnMilwaukee LLC, I (we) agree to abide by the rules and regulations at this event.
I (we) agree that Milwaukee Radio Alliance LLC and OnMilwaukee LLC has the right to refuse this entry for any cause.
I (we) agree to hold Milwaukee Radio Alliance LLC and OnMilwaukee LLC, its owner(s), members, directors, governors, officers, agents or other functionaries, any employees of the aforementioned parties and the owner(s) of the premises or grounds harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of my dog(s) while in or upon the Lure Course, Ball Pit, and Off-Leash areas, the Bark & BBQ grounds or near any entrance thereto and I (we) personally assume all responsibility and liability for any such claim, and I (we) further agree to hold the aforementioned parties harmless from any claim, loss of my dog(s) by disappearance, theft damage or injury be caused or alleged to be caused by the negligence of Milwaukee Radio Alliance LLC and OnMilwaukee LLC, or any of the aforementioned parties or by the negligence of any person or any other cause or causes. I (we) certify and represent that the dog(s) entered is not a hazard to persons or other dogs.
Furthermore, Milwaukee Radio Alliance LLC and OnMilwaukee LLC reserves the right to stop or limit any dog’s participation in a practice or event due to age, body condition, physiological, temperament, weather conditions, or any other factor that could affect the safety of the dog(s), other participating dogs, or spectator.
I (we) have read this Release of Liability and understand that I have given up substantial rights, including the right to sue in the event of death, injury, or loss. I acknowledge that I am voluntarily signing this agreement, and intend my signature to be a complete release of all liability, including that due to inherent risks and/or the ordinary negligence of the Protected Parties, to the greatest extent allowed by law of the State of Wisconsin.
